Free printable pet emergency card

If you are in an accident or taken to hospital, the people helping you may not know a dog or cat is waiting at home. A card in your wallet tells them, and tells them who to call.

Nothing you type here leaves this page. It is not saved or sent anywhere. Fill it in, print it, and close the tab. Leave any field blank to write it in by hand.

What a card cannot do

A card helps only if somebody finds it and has time to read it. It cannot help if nobody knows anything has happened to you, which is the common case when you live alone: a fall at home, a sudden illness, an accident on a walk without your wallet. Our guide to what happens to pets home alone in an emergency covers the rest of a plan, and the pet emergency plan guide walks through choosing the people on this card.

Where Woof fits

Woof covers the case a card cannot. You check in on a schedule you choose. If you stop responding, Woof contacts the people you picked, so someone knows your pet is waiting even if nobody found your wallet. It never calls 911 on its own: it tells your people.
